How to fill out forms on PC

Click the PDF form, it will open in a new tab.

Download the file and you can either open it on your PC which you could fill out from there, or might open in the browser tab.

Fill out the form. However, to keep the information you just filled out..

Click ctrl+p or click the printer icon to bring up the "Print window"

Instead of printing, (unless you'd prefer to print and fill out) change the printer option to save as a PDF

You're done! Now your information is saved to the PDF file on your computer and you can send them to me during your doxy telecommunication appointment so that they can be securely sent through that platform.
